On Thursday, Cardiff announced that Moore, who began his career at the Bluebirds in 1957, had passed away at the age of 74.
However, ahead of the Welsh club's fixture against Charlton Athletic at The Valley, a planned tribute to Moore went wrong after a photo of Alan Campbell, who played for Cardiff between 1976 and 1980, was screened instead.
A statement from Charlton read: "The club would like to apologise to supporters and the family of Graham Moore after the wrong image was displayed prior to kickoff.
"The correct image was displayed and an apology read out at half time. The family have also been apologised to personally."
The incident marked the start of a disappointing afternoon for Charlton, who were held to a goalless draw in front of their own supporters.
They now sit six points adrift of safety in the Championship after Fulham, Bristol City and MK Dons all recorded three points on Saturday afternoon.
