The goalkeeper once again faltered for the Reds at Anfield.
But a few all-too-familiar events denied the Reds an important victory in the Premier League. Poor set-piece defending was one and a blunt display from Christian Benteke and a few of his team-mates was another.
For spells, Liverpool were brilliant against the Baggies. They started well and finished even better. But, for large spells, the hosts were also just as bad - while West Brom executed a near-perfect game plan to stifle the home side.
The biggest disappointment for the Merseyside outfit was, however, Simon Mignolet.
Indeed, in an unsurprising turn of events, the Belgian burst towards the ball from a corner yet failed to collect it in the air. He flapped about, looked hesitant and, bang, West Brom netted their equaliser, before going on to later score from another set-piece.
Liverpool fans reacted unforgivingly on Twitter. Many are fed up of the goalkeeper's erratic mistakes and lack of authority in the penalty area. And many want him gone completely...
